The Children’s Long-Term Support (CLTS) Program is a Wisconsin program that helps children and young adults with disabilities live at home and stay involved in their communities. It uses Medicaid funding to pay for supports that help families meet daily needs, reduce stress, and improve quality of life. Each family works with a support and service coordinator who helps identify the child’s needs, create a personalized plan, and connect the family with services. These services can include a variety of resources that help children live safely and comfortably at home, including YogAutism sessions.
Children and young adults may be eligible for CLTS if they are under age 22, qualify for Wisconsin Medicaid, and live in an approved home or community-based setting. They must also have a certain level of developmental, physical, emotional, or mental health needs. Families must also be willing and able to receive services in their home or community.
To enroll in CLTS, families start by contacting their county’s human services or disability services agency. A county worker will meet with the family to explain the program, gather information, and complete a “functional screen” to assess the child’s needs. If the child qualifies, the family and coordinator create an individualized service plan that outlines which supports will be provided. There is no longer a statewide waitlist, so eligible children can begin receiving services as soon as they are approved. Once enrolled, families continue working with their coordinator to access services and update the plan as needs change.
The overall goal of CLTS is to make sure children with disabilities can remain with their families, take part in everyday life, and get the support they need to thrive in their own homes and communities.
